Monitoring Tool for Home Sharing
CLBC uses the Monitoring Tool for Home Sharing to monitor directly contracted home sharing arrangements. Agencies are expected to meet or exceed the rigour of this tool in their monitoring process. The Monitoring Tool for Home Sharing promotes the practice of continuous quality improvement. This tool reviews the 15 standards in the four domains.
Typically, the quality service analyst or agency coordinator and the home sharing provider complete the monitoring tool together. Ideally, the individual being supported by a home sharing provider should have the opportunity to provide feedback and participate in this review. Assistants, such as respite providers, may also be invited to participate.
The Monitoring Tool for Home Sharing is accompanied by a detailed action plan. The action plan identifies each goal for improvement, the person responsible for making the improvement, and the target date for completion.
