Quality of Life Report for Home Sharing
In Module 1 you examined the Quality of Life framework and discovered that you are contractually obligated to incorporate quality of life outcomes into the support you provide to the individuals with whom you share your home. How you are fulfilling this part of your contract is documented in the Quality of Life Report for Home Sharing.
Home sharing providers must submit a Quality of Life Report for Home Sharing at least twice a year: during the week of January 1st and during the week of July 1st. Your agency may request additional reports.
This report documents a number of areas including:
- Weekly schedule
- Goal planning
- Health and well-being
- Unresolved issues
- Community inclusion
This report compliments other monitoring tools and helps provide a comprehensive picture of the support being provided to individuals in home sharing arrangements.
