File a Complaint

Community Living BC’s complaint process is designed to be accessible, person-centred, responsive and fair. In addition to being easy for you to use, it features clear timelines you can rely on and identifies a responsible CLBC manager at each stage of the process.

You have three options to submit your confidential complaint:

1. Click here to complete the on-line Complaints Resolution Form. When the form is completed, click the “Submit” button at the bottom of the page.

2. Click here to download the Complaints Resolution Form which you can print, fill out and fax or mail to CLBC.

3. If you do not wish to file your complaint on-line, by fax or by mail, you can contact the CLBC Quality Assurance office directly at 1-855-664-7972. A member of our staff will take your information over the phone.
