Community, Family and Caregiver Education and Training
PAC uses the following approaches when providing caregiver training:

· Observation and involvement throughout transitioning process
· Analysis of individual’s adaptation to change via group interaction and response to limits being established
· Participation in dialog pertaining to program direction
· Interaction with the teams during planning and relationship development
· Facilitation of enhanced skill development through information exchange, negotiation, and application of recommendations

Community Planning

Through collaboration, PAC assists in individualized planning utilizing the following strategies:

· Championing the richness of each individual’s care assistance network
· Facilitating information sharing
· Focusing on strength-based planning
· Assisting in the development and implementation of measurable goals
· Clarifying team and individual responsibilities and accountabilities
· Providing diagnostic review, treatment and update
