In-Home Assessment
From the time of referral, under the direction of the community outreach supervisor, the PAC begins outreach work in the individual’s community. The first assessment occurs in the individual’s home with the full involvement of the individual, their care-giver and mental health worker.

The community outreach supervisor will organize follow up after discharge. This involves PAC staff revisiting the individual in their home/community environment, assessing progress, reviewing personal goals against achievements, and providing further recommendations.
