Please keep your copy of the completion confirmation and provide your agency with a photocopy.
Privacy breaches or unauthorized disclosure of personal information, related to services or supports for people who are funded by CLBC, must be reported to a CLBC analyst. Please refer to your agency’s policies for direction about who in your organization should make the report to CLBC, and how they should make the report.
