When an Information Incident Occurs

You care about the individuals that you support and would do anything to protect their personal information. But if you feel an information incident may have occurred, you need to act fast and follow the four steps.

Step 1: REPORT

Immediately contact your local CLBC office to report if there may have been a privacy breach or information incident. Don’t worry—you won’t be judged. CLBC only wants to help assess and lessen the impact of the situation.

If possible, provide:

  • The date of the incident
  • What information may have been involved
  • Who was involved
  • Whether any information has been recovered

Contact the police to report stolen or lost electronic equipment. The local CLBC office will advise you about the steps that follow.

Step 2: RECOVER

Attempt to regain the information if the recovery is straightforward and the recovery will not cause further harm. Prevent further unauthorized use or minimize the potential for unauthorized use of information or records.

Step 3: REMEDIATE

Work with CLBC to determine what went wrong and the possible extent of the damage. Only if directed by CLBC, contact those who may be harmed by the information incident or privacy breach.

Step 4: PREVENT

After investigating the incident, put into practice changes that will reduce future information incidents.
