Sharing: Request for Information

CLBC

• CLBC may request records about individuals receiving CLBC services from any service provider at any time. Follow the directions given by the local CLBC office.
• If CLBC receives a Freedom of Information (FOI) request, CLBC may ask for service provider records pertaining to the individual that match the request. Any requests for information have legislated deadlines and service providers must be prompt with providing the required records to their local office.

As a service provider, you may be asked to share information with various individuals or organizations with the individual’s consent.

 

 

Individuals and Their Legal Representatives
Under FOIPPA, individuals have the right to:
• Access their personal information
• Correct their personal information
