Best practice is to have your organization’s records for the individuals up to date and accessible. Be ready for requests for records.
If the information request comes from the individual or their legal representative, and the request is for a large number of records or you feel the information needs to be checked for information that could be harmful if released, contact CLBC before proceeding.
If the information request comes from the individual or their legal representative, and is low risk (i.e., their personal plan) and does not contain anyone else’s personal information, it is okay to provide the information. However, information cannot be released to anyone else, including the family, unless a consent is on file and is current.
Corrections to personal records received from individuals or their representative or committeeship are to be directed to CLBC. Corrections to personal records are to be completed within five business days after CLBC contacts the service provider.
By law, public bodies and organizations providing government-contracted services must assist individuals if they request their information or would like to change their information.
